Referral Information
Referrals to Pine Grove can be made to the Director of Admissions, Donna Moen. Initial referrals can be verbal or written. Official referrals must include a Children's Services Universal Application, and historical and evaluative information (including a social history, psychological and psychiatric information and medical and educational history.)
